We continually register home caregivers for referral through our organization. To become a registered caregiver with Northwest Home Care, Inc., please follow the instructions below.
To Register as a Caregiver for Northwest Home Care,Inc. please request a "Caregiver Packet" from mail@nwhomecare.com Be sure to give your correct mailing address.
Return the completed application to us with the completed reference sheet. Remember- your references MUST BE previous places of work. We will check your references and perform a criminal and motor vehicle background check. If all information is satisfactory, we'll call you for an interview. |

1) U. S. Driver's License (or valid picture ID if not a driver)
2) Social Security Card
3) Nursing License and Insurance Card (if applicable)
4) CNA Certificate (if applicable)
5) Home Health Aide Certificate (if issued and applicable)
6) CPR Card (current or expired, if you have CPR training)
7) Resident Alien Card/Passport/US work authorization (if not a US Citizen)
8) Any other certifications you may have dealing with health care
